This violation was observed on or about August 03, 2012, when employees were exposed to fall and crushed-by hazards while working on the second level of a scaffold that was partially set up on a dirt foundation without base plates or mud sills.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: The employer does not ensure that a guardrail system is installed on all working levels of scaffolds to prevent employee injury. This violation was observed on or about August 03, 2012, when employees were exposed to a fall hazard while on the second working level of a scaffold without a guardrail system in place. H & J Plastering, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 314338849, citation number 01 item number 02 and was affirmed as a final order on 11/22/2010, with respect to a workplace located at 10222 East Freeway, Jacinto City, TX 77029.
