29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Jobsite, Old Frankstown Road, Monroeville, PA: On or about 08/03/12 employees working on a low-sloped roof approximately 20 feet above the ground were not protected from falling by the use of guardrail systems, safety net systems, personal fall arrest systems, a combination of warning line and guardrails/safety net or personal fall arrest system or a warning line system and safety monitoring system. Employees were observed directly at the edge performing work with no fall protections systems or monitoring systems in place.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill: (a) Jobsite, Old Frankstown Road, Monroeville, PA; On or about 08/03/12 retraining had not been conducted when inadequacies in the affected employee's knowledge or use of fall protection systems indicated the employees did not retain the requisite understanding or skill needed to protect themselves from fall hazards. Employees were observed working at the edge of the roof, exposed to a 20 ft. fall hazard, and no fall protection systems were in place to protect the employees.
