29 CFR 1926.503(c): When the employer had reason to believe that any affected employee who has already been trained did not have the understanding and skill required by paragraph (a) of this section, the employer did not retrain each such employee:    a) Jobsite, 531 Clairton Boulevard, Pleasant Hills, PA: On or about July 24, 2014, during construction activities, the employer did not ensure that employees were retrained in proper fall protection practices where exposed to falls of approximately 12 feet and appropriate fall protection systems/devices were not used.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) Jobsite, 531 Clairton Boulevard, Pleasant Hills, PA: On or about July 24, 2014, during construction activities, the employer did not ensure that the extension ladder, used to gain access to/egress from the roof, extended at least 3 feet above the roof line, exposing employees to fall hazards of up to approximately 12 feet.
