OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to struck-by, caught-in, and/or amputation hazards: (a) In the CNC area for employees operating the Supermax Max 3, Rebel CNC machine, where the CNC machine was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the machine center or being caught in the moving machinery by opening the machine door during the operating cycle. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install a door interlock device on the Supermax Max 3, Rebel CNC machine as required by ANSI B11.2-2002, Requirements for Turning Centers Automatic Numerically Controlled Turning Machines, paragraphs 6.21 Safeguarding and 6.21.1.2 Automatic Mode which states: 1. Paragraph 6.21: Machines shall be safeguarded wherever practical by fixed or interlocked moveable guards. 2. Paragraph 6.21.1.2: Machine operation shall only be possible when the guards are closed and the safeguarding (protective) devices are active. If it is possible, in this mode, to open an interlocked moveable guard, the hazardous movement shall cease and be inhibited.

29 CFR 1910.303(b)(7)(i):     Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment    Welding area across from main electrical service room, Southbridge Sheet Metal Works;  480 volt electrical disconnect box DS 7 at the welding station had a missing knockout that was greater than 1 inch in diameter that would allow employees to access the energized parts exposing them to electric shock and electrocution hazards.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    Facility wide, Southbridge Sheet Metal Works; Employees used extension cords for a variety of reasons such as powering fans and person radios as well as providing power to computers and other work related tools, however, the cords were not inspected to check for damage to the cord prior to use exposing the employees to electric shock and burn hazards as well as fire hazards.

29 CFR 1910.184(d):     Damaged or defective sling(s) were not immediately removed from service:      Welding area across from main electrical service room, Southbridge Sheet Metal Works; Employee was using damaged sling to move machined metal piece to work table exposing the employee to struck by hazards and laceration hazards.    Southbridge Sheet Metal Works, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.184(d) which was contained in OSHA inspection number 315514745, citation number 1-3a and 1-3b, issued on September 15, 2011, with respect to a workplace located at 441 Main Street, Sturbridge, Massachusetts. The final order date for the citation was on October 6, 2011.
