29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. An employee working from an aerial lift were exposed to falls at approximately 15 feet to a lower level, employee were not protected by the use of a body harness. The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). An employee working from an aerial lift were exposed to falls at approximately 15 feet to a lower level, employee were not protected by the use of a body harness.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: An employee working from an aerial lift were exposed to falls at approximately 15 feet to a lower level, employee were not protected by the use of a body harness. The employer did not provide training to his employee who might be exposed to fall hazards.
