29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about August 7, 2012, at the above addressed jobsite, an employee was installing a water line in a trench measured at seven (7) feet ten (10) inches in depth. The trench was not provided with cave in protection, thereby exposing the employee to cave-in hazards. THE GERARDI SEWER & WATER CO., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314036377, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 2, 2012 WITH RESPECT TO A WORKPLACE LOCATED AT 400 BLOCK HOWARD ST. ELMHURST, ILLINOIS 60126. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
