On or about June 11, 2018, employees were exposed to cave-in hazards as they worked in an excavation approximately 7.3 feet deep on the north face of the excavation and approximately 6.5 feet deep on the south face of the excavation without cave-in protection.      Abatement documentation is required for this item in accordance with requirements of 29 CFR 1903.19(d).
