Instance #2     On or about 8/1/2012, Sugar Bay Resort & Spa, 6500 Estate Smith Bay, St Thomas  Main Kitchen / hallway refrigerator area:  Employees were using an electrical refrigerator  where its compressor lid cover was missing, therefore, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact.
