29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.    On or about August 8, 2012, employees were observed working from the top level of a blue mobile scaffold that was not fully planked. Employees were exposed to fall hazards from a height greater than 10 feet to the concrete surface below.

29 CFR 1926.451(g)(1)(vii):   Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.      On or about August 8. 2012, for the blue mobile scaffold on the northwest corner of the building project, an employee was performing work from a scaffold without being restrained from a fall by a guardrail system or by a personal fall arrrest system.  The employee was exposed to fall hazards at a distance greater than 10 feet to the concrete surface below.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.        On or about August 8. 2012, for the blue mobile scaffold on the northwest corner of the building project, a ladder was not provided for accessing the scaffold.  The employee climbed up the scaffold components to access the working level and was exposed to fall hazards at a distance greater than 10 feet to the concrete surface below.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation.    On or about August 8, 2012, for the blue mobile scaffold at the northwest corner of the building project, a caster and wheel had been removed from an upright and the upright was positioned on a piece of scrap wood without a base plate being installed on the upright.  Employees were exposed to a fall hazard approximately 10 feet to the concrete surface below.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.     On or about August 8, 2012, for mobile scaffolds on the site, no employee had been qualified to erect, dismantle, or alter scaffolds.  Employees were exposed to fall hazards resulting from deficiencies such as, but not limited to, a missing base plate, lack of a guardrail system or issuance of personal fall arrest systems, and working levels not being fully planked.
