(a) Ambulance Building, Roof - On or about August 9, 2012 employees were removing roofing rubber and insulation from a flat roof and were not protected from falling approximately 15 feet to grade. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: (a) Ambulance Building, Roof - On or about August 9, 2012 the employer installed two anchorage points which were connected to non-structural building components including rubber piping and metal vents as part of a personal fall arrest system which was not capable of supporting 5000 pounds per employee. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

WAS CITED PREVIOUSLY FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.501(b)(1) OR ITS EQUIVALENT STANDARD , WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314155565 , 01, 01 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 3, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 675 RED DALE ROAD, ORWIGSBURG, PENNSYLVANIA 17961. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
