29 CFR 1926.101(a): Ear protective devices were not provided and/or were not used when it was not feasible to reduce noise levels or duration of exposures to those specified in Table D-2, Permissible Noise Exposure, in 29 CFR 1926.52: Anderson Air Force Base, Yigo, Guam: On or about July 30, 2012, and at times prior thereto, the employer failed to require that an employee operating a jackhammer HILTI TE 1000-ARV wear the hearing protection provided by the employer.

29 CFR 1926.51(a)(5): Where single service cups (to be used but once) were supplied, both a sanitary container for the unused cups and a receptacle for disposing of the used cups were not provided. Anderson Air Force Base, Yigo, Guam. On or about July 30, 2012, and at times prior thereto, the employer failed to provide a sanitary container or a waste receptacle near the drinking water station exposing workers to drinking from dirty and contaminated previously used single service paper cups.
