29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.      On August 08, 2012, and times prior to, saw dust had accumulated in large amount on the floors of the facility. This condition was present throughout the wood working area of the facility exposing employees to trip and fall hazards and potential fire hazards.

29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route.     On August 08, 2012, and times prior to, employees were working inside the building with an emergency exit that was blocked.   This condition exposed employee to potential fire hazards.

29 CFR 1910.178(f)(1):  The storage and handling of liquid fuels such as gasoline and diesel fuel were not in accordance with NFPA Flammable and Combustible Liquids Code (NFPA No. 30-1969), which is incorporated by reference as specified in Sec. 1910.6.     On August 08, 2012 and times prior, South side of the facility, three LPG tanks used to provide fuel for the powered industrial trucks were not secured nor stored as per NFPA Flammable and Combustible Liquids Code requirements.  This condition exposed employee to possible burns in case of an explosion and/or stuck by hazards.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage.     On August 08, 2012, and times prior to, in the main shop, the commercial fabrication area, and the paint booth area, the electrical panels used to control power to the building and other machinery were blocked.  In the event of an emergency employees would not have safe access to turn off power to the building.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective.      On August 08, 2012 and times prior to, employees were using a fan that did not have proper grounding therefore energizing the entire outer shell of the fan.  This condition exposed employees to electrical shock.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.      On August 08, 2012, and times prior to, employees were using extensions cords without proper tension control devices such as strain reliefs. Cords were suspended from the ceiling and were used to power tools.  This condition exposed employees to electrical shock.
