29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which was incorporated by reference as specified in 29 CFR 1910.6:    (a)  B & G Department - On or about 5/10/12 and at times prior thereto, a nitrogen compressed gas cylinder was missing a cap and subject to being knocked over or damaged.  The cylinder was not in use.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:      (a)  B & G Department - On or about 5/10/12, and at times prior thereto, a Wissota double wheeled bench grinder; the left wheel had a measured 3/8" gap between the wheel and the work rest and the right wheel had a measured 1/2" gap between the work rest and the wheel.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:    (a)  B & G Department - On or about 5/10/12, and at times prior thereto:    1.  Brown-Brockmeyer double wheeled bench grinder was missing both tongue guards.  2.  Wissota double wheeled bench grinder was missing one tongue guard on the right side.

If valves cannot be opened by hand, the supplier shall be notified:    (a)  B & G - On or about 5/10/12, and at times prior thereto, it was observed that a wrench attached to the stem of an acetylene cylinder was used to control gas flow instead of an approved valve.
