On or about August 8, 2012, Roof of Building, Fall protection was not used at the north side of the roof where materials were lowered to the ground by rope and bucket and where the parapet was less than 42 inches in height which exposes employees to falls from elevations greater than 6 feet.

On or about August 8, 2012, 2nd Floor of Building, An extension ladder was set-up directly behind a guardrail for a floor hole which allows employees to walk to the floor hole and exposes masonry employees to falls greater than 6 feet in height.
