29 CFR 1926.1053(a)(13): The minimum perpendicular clearances between fixed ladders' rungs, cleats, and steps, and any obstructions behind the ladders were not 7 inches (18 cm), except in the case of an elevator pit ladder for which a minimum perpendicular clearance of 4 1/2 inches (11 cm) is required:    At the east side of the project: On August 15, 2012, and at times prior thereto, the fixed ladder used to access the roof had a 2 inch clearance from rungs to the concrete exterior wall behind the ladder.  It was about 10 feet from the roof to the concrete balcony below.
