29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) Schmucker Framing LP worksite located at 6016 Lilac, Monclova, Ohio: On or about August 17, 2012, the employer did not ensure employees wore proper eye protection while operating  pneumatic nailers while installing plywood sheathing and performing framing activities on the two story residential structure. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    A. Schmucker Framing LP worksite located at 6016 Lilac, Monclova, Ohio: On or about August 17, 2012, the employer did not ensure employees were protected from head injuries while working below employees installing plywood sheathing on the east side of the two story residential structure. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:    A. Schmucker Framing LP worksite located at 6016 Lilac, Monclova, Ohio: On or about August 17, 2012, the employer did not ensure employees were protected from falls while installing roof sheathing on the east side of the two story residential structure.  The employees were exposed to a fall distance of approximately 18.00 feet from the 8/12 pitch roof.    B. Schmucker Framing LP worksite located at 6016 Lilac, Monclova, Ohio: On or about August 17, 2012, the employer did not ensure an employee was protected from falls while performing framing activities on the north side of the two story residential structure.  The employee was exposed to a fall distance of approximately 10.00 feet while walking on the exterior top plate.
