The employer shall ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards. On or about 08/04/2012, an employee who was not wearing protective foot wear was exposed to struck by / crushing hazards while pushing a forklift whose wheel ran over the foot of the employee at facility located at 2900 NW 35th Street, Miami FL. 33142.
