29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts and in-running nip points: (a) On the #40 APV Baker extruder, an inadequately guarded rotating purging screw exposed amputation hazards for employees adding purging material to the machine. (b) On the #40 APV Baker extruder, the rolling pins used to flatten product were not guarded, exposing employees to amputation hazards. (c) On the #40 APV Baker extruder, the kibbler roller guard had 2 1/8 inch openings at a distance of 6 1/2 inches from in-running nip points, exposing employees to amputation hazards. (d) On the Baker Perkins extruder #65-1, an inadequately guarded rotating purging screw exposed amputation hazards for employees adding purging material to the machine. (e) On the Baker Perkins extruder #30-1 extruder, an inadequately guarded rotating purging screw exposed amputation hazards for employees adding purging material to the machine.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): (a) The employer did not ensure energy control procedures specified the location of the disconnect(s) and the method of verifying energy isolation from equipment, such as but not limited to the Five Gallon Filling Machine.
