29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) On or about February 22, 2019, the employer did not ensure Small Batch Line grinder (Model No. K300Z) machine specific procedures were utilized by employee(s) when cleaning the surface of stationary components of the system, in that the auger was not de-energized and was rotating.  This exposed the employee(s) to ingoing nip point, crushing hazard and amputation injury.    In the alternative...    29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and crushing hazards.    (a)  On or about February 22, 2019, the employer did not insure barrier guarding, interlock and/or light curtains were properly implemented as part of the integrated safety system for the Small Batch Line grinder (Model No. K300Z).  This allowed the employee(s) to access the rotating auger in an energized state, and exposed the employee(s) to ingoing nip point, crushing hazard and amputation injury.
