29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    On or about 08/17/2012, an employee was working from an aerial lift without being tied off to the basket, without having the bottom part of the harness on, and half the shoulder strap off.  The employee was exposed to a fall hazard of approximately 15 feet to the ground.
