29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At the worksite, on August 17, 2012, employees were using the Makita model number 5007N, S/N 0802100 to cut wood and hardy plank. The flexible cord supplying power to the saw had exposed inner conductors at tjhe terminal end, exposing employees to electrical shock.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: At the worksite, on August 17, 2012, employees were repairing the window frames and hardy planks from the fabricated frame scaffold. The employees used the crossbraces and vertical posts as a means of access. This condtion exposed employees to falls of apposimately 21 feet.
