This violation most recently occurred on August 14, 2012, in the nailing room, where the employer did not ensure that employees using pneumatic nailers to secure pallets wore appropriate eye protection.

29 CFR 1910.147(c)(1): Energy control program. The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machines or equipment were not isolated from the energy source and rendered inoperative.    This violation most recently occurred on August 14, 2012, in the nailing room, the employer did not develop and implement hazardous energy controls programs when servicing and maintaining machines and equipment including, but not limited the Viking pallet nail machine.

This violation most recently occurred on August 14, 2012, in the nailing room, at least one employee, who operated the nailing machine, was not protected from contracting the in-going nip-point created between the drive chain.

29 CFR 1910.334(2)(ii) The employer did not ensure that defected or evidence of damaged that exposed an employee to injury ,the defective and damaged item shall be removed from service, and that no employee should use it until repairs and test necessary to render the equipment safe had been made.    This violation most recently occurred on August 14, 2012, in the nailing room, where the employer did not ensure that employees using, 120V-15A-7  circular saw cord that had worn and frayed electrical cord, exposing bare electrical conductors.
