29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by unguarded points of operation: On or about August 17, 2012, employees were required to operate the following band saws with the unused portions of the blades unguarded: 1. Marvel vertical band saw #51 (Final inspection area) 2. Marvel vertical band saw #50 (Maintenance area) The employees were thereby exposed to the hazard associated with reciprocating saw blades.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a - Final inspection area - On or about August 17, 2012, an employee experienced an amputation injury while performing a cut on a metal forge with an inadequately guarded vertical band saw. Employees were thereby exposed to hazards associated with unguarded points of operation.
