29 CFR 1910.179(j)(2)(i): Frequent Inspection. All functional operating mechanisms for maladjustment interfering with proper operation were not conducted daily. a. Steel Yard Building/Billets - On or about October 16, 2012, the employer did not conduct daily inspections of the Milwaukee 15 Ton crane. The operating mechanism for the crane's hook had a damaged safety latch. Employees were thereby exposed to struck by hazards.

29 CFR 1910.219(b)(1): Flywheels. Flywheels located so that any part is seven (7) feet or less above floor or platform shall be guarded in accordance with the requirements of this subparagraph: a. KFA Building/Forge Shop - On or about October 16, 2012, the metal woven type guard on the lower portion of the flywheel located at less than seven (7) feet from the ground was loose and falling apart from its frame. Employees were thereby exposed to the hazards of rotating parts.
