Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about August 15, 2012, BNSF Railway Company did not assure by means of a post assembly inspection by a qualified person that the Manitex model 30102C had all outriggers and stabilizers fully extended as required by the load chart. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about August 15, 2012, BNSF Railway Company employees operated a Manitex model 30102C S/N 158391 in excess of its rated capacity causing the machine to overturn. The rated capacity of 2932 lbs. includes deductions (47 degree boom angle) was exceeded by the weight of the signal bridge (load weight 3540 lbs.) Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

In addition, when requested by the operator, this information must be provided to the operator prior to the lift; or On or about August 15, 2012, BNSF Railway Company did not provide the weight of the signal bridge to be hoisted to the on site crane operator. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
