29 CFR 1926.21(b)(2):     The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a) Construction Site:  On or about August 16, 2012 and at times prior thereto, employees were exposed to an ejection and fall of approximately 25 feet to the ground level below, while installing new roofing material from a Genie aerial lift.

29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a) Construction Site:  On or about August 16, 2012 and at times prior thereto, employees were exposed to an ejection and fall of approximately 25 feet to the ground level below, while installing new roofing material from a Genie aerial lift.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:     (a) Construction Site:  On or about August 16, 2012 and at times prior thereto, employees were exposed to a fall of approximately 25 feet the ground level below while installing new roofing material on a 4:12 pitch roof.
