29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. a) Roof of Classroom Building, Front Side, Samuel Donald School: Employees engaged in the installation of a roof surface did not have acceptable means of fall protection; thereby, were exposed to a fall hazard approximated to be 35' above ground surface below, on or about 8/20/2012. b) Roof of Classroom Building, Side of Building, Samuel Donald School: Employees engaged in the installation of a roof surface did not have acceptable means of fall protection; thereby, were exposed to a fall hazard approximated to be 35' above ground surface below, on or about 8/20/2012. c) Roof of Gymnasium, Samuel Donald School: Employees engaged in the installation of a roof surface did not have acceptable means of fall protection; thereby, were exposed to a fall hazard approximated to be 15' above a lower roof surface, on or about 8/20/2012.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed. a) Side of Classroom Building, Samuel Donald School: Employees used a 40' Werner aluminum extension ladder to access school roof surface. Ladder rails extended approximately 6" above upper landing surface, on or above 8/20/2012.
