29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l) and as referenced in the note contained in 1926.602(d):    a) Jobsite, Butler Area Sewage Authority, 122 Etna Street, Butler, PA: on or before 08/13/12, the employer did not certify that the operator of a Gradall Rough Terrain forklift truck, SN0190324, Model 66-42A had been trained and certified in the operation of the powered industrial truck.    Pacifico Construction Company was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 29 CFR 1910.178(l)which was contained in OSHA Inspection number 314113713, Citation number 1, Item number 001, issued on 04/25/11, with respect to a workplace located at 144 Westerly Treatment Plant Road, Duncansville, PA
