The employer did not ensure that back of the scaffold was provided with a guard rail and employees were exposed to a fall of 23 feet from the scaffold platform to the bottom of the empty sewage tank on or about September 11, 2014.  (b) Inside Pump House Lower Level - Two employees worked from a job made large area supported scaffold to apply and sand dry wall joint compound to the ceiling.  The south edge of the scaffold was not provided with a guard rail and the employees were exposed to a fall of 11 feet 8 inches on or about September 11, 2014.   No abatement certification or documentation required.
