29 CFR 1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall be rigged such that employees can neither free fall more than 6 feet, nor contact any lower level: (a) VMC Roofing Company at 6146 Chesney Ct, Windsor, CO 80550: On and before August 20, 2012, the exposing employer did not ensure that employees were protected from injury, in that two employees installing shingles on a 5-in-12 pitch roof on a two-story residential home did not utilize fall protection systems as designed. Each employee utilized a separate fall harness, 40-foot lifeline, and rope grab assembly. One end of the lifeline was connected to the D-Ring on the fall harness and the other end of the lifeline, including the rope grab assembly, was wrapped around a roof truss and then was connected back onto itself. This condition exposed employees to fall hazards of approximately 25 feet.
