(a) 302 Deer Path Drive - On or about August 17, 2012 employees were working from a supported scaffold platform which was damaged and the width exceeded the wheelbase of the Pettibone 8044 forklift supporting it. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.602(c)(1)(vi): All industrial trucks in use shall meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks; a safety platform firmly secured to the lifting carriage and or forks was not used: (a) 302 Deer Path Drive - On or about August 17, 2012 an employee was elevated and worked from the forks of a Pettibone 8044 rough terrain forklift to install wood sheathing and was not protected from falling approximately 16 to 18 feet to grade. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
