29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) Rooftop - Employees using air nail guns were exposed to struck-by hazard while nailing without the use of eye or face protective equipment, on or about April 17th, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) 5502 Main Street - Employees setting trusses from a rooftop were exposed to a fall hazard of approximately sixteen (16) feet without the use of a guardrail system, safety net system or personal fall arrest system, on or about April 17th, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. Keystone Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1)), which was contained in OSHA inspection number 314246182, citation number 1, item number 1 and was affirmed as a final order on (07/22/2011), with respect to a workplace located at 600 East Roseville Road, Lancaster, PA 17601.
