A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.    Throughout Facility - On or about August 21, 2012, the employer did not provide employees with an exit route door that remained unlocked, thereby exposing employees to smoke inhalation and burns.

29 CFR 1910.37(a)(3):     Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.    Throughout facility - On or about August 21, 2012, exits routes were not continuously maintained free of recycled parts, motors and transmissions, thereby exposing employees to obstructed means of egress in the event of an emergency.

29 CFR 1910.37(b)(6):     Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 foot lamberts (0.21 cd/m2) are permitted.    Throughout facility - On or about August 21, 2012, the emergency exit sign was not illuminated by a reliable light source, thereby exposing employees to burns and smoke inhalation.

29 CFR 1910.110(e)(4)(iii):     Permanent and removable fuel containers shall be securely mounted to prevent jarring loose, slipping, or rotating, and the fastenings shall be designed and constructed to withstand static loading in any direction equal to twice the weight of the tank and attachments when filled with fuel using a safety factor of not less than four based on the ultimate strength of the material to be used. Field welding, when necessary, shall be made only on saddle plates, lugs or brackets, originally attached to the container by the tank manufacturer.    Throughout facility - On or about August 21, 2012, the LP gas cylinder was not securely mounted on the Mitsubishi and Hyster forklift trucks, in that the mounting latches were damaged or missing, thereby exposing employees to struck by, fire, and/or explosion hazards.
