The working platforms were not fully planked and consisted of three planks each.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: On 08/21/2012, at an establishment located at 3604 Bright Star Way, Plano, Texas; an employee standing on the second tier of a tubular welded frame scaffold while staging masonry bricks was exposed to a fall hazard. The scaffold was not provided with guardrails and the employee did not wear personal fall protection.
