29 CFR 1926.416(e) (1): Worn or frayed electric cords or cables were used. On or about August 23, 2012, employees engaged in framing activities at a residential building under construction located at 509 Emerald Green in El Paso, TX, were utilizing worn out electric cords to power their power tools. This condition exposed employees to potential electrical shock and/or burn hazards.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure: On or about August 23, 2012, at a residence roof located at 509 Emerald Green, in El Paso TX, employer did not ensure that employee(s) working on the roof were provided with and required to use adequate fall protection. Employees were exposed to fall hazards from and elevation of approximately 10 feet above ground level.
