29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept clear from work areas, passageways, and stairs, in and around buildings or other structures: On or about July 10, 2015, the employer did not ensure that trash, scrap lumber, scrap lumber with protruding nails, and other debris were kept clear from the work area. This condition exposed employees to impalement and tripping hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about July 10, 2015, the employer did not ensure that employees were protected from falling to ground level by any means. This condition exposed employees to fall hazards of 6 feet and above. Puentes Construction was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 593620, citation number 01, item number 002 and was affirmed as a final order on August 23, 2012, with respect to a workplace located at 509 Emerald, El Paso, TX 79928.
