On or about August 20, 2012, on the east side of the apartment building, the welding cable being used to weld structural beams had several nicks and cuts on the ground side of the welder, exposing employees to an electrical shock hazard.

On or about August 20, 2012, on the 2nd floor of the apartment building, near the job made ladder access there was a 7-inch x 70-inch rectangle hole unprotected, exposing employees to tripping hazard.    b. On or about August 20, 2012, on the 3rd deck floor of the apartment building, there was a 12-inch x 12-inch square hole unprotected, exposing employees to tripping hazard.    c. On or about August 20, 2012, on the 4th deck floor of the apartment building, there was an 8-inch x 8-inch square hole unprotected, exposing employees to tripping hazard.

On or about August 20, 2012, on the 5th floor of apartment building, 3 employees were setting rebar and concrete forms near an unprotected edge without the use of guardrail systems, safety nets, or personal fall arrest, exposing themselves to a 53-foot fall hazard.    b. On or about August 20, 2012, on the 4th floor of the apartment building, 2 employees were setting concrete form shoring near an unprotected edge without the use of guardrail systems, safety nets, or personal fall arrest, exposing themselves to a 42.4-foot fall hazard.    c. On or about August 20, 2012, on the 3rd floor of the parking garage, 3 employees were cutting rebar and pulling concrete forms near an unprotected edge without the use of guardrail systems, safety nets, or personal fall arrest, exposing themselves to a 31.8-foot fall hazard.    d. On or about August 20, 2012, on the 3rd floor of the parking garage and administrative office spaces, employees were clearing rebar and rubble to set concrete tunnel forms near unprotected edges without the use of guardrail systems, safety nets, or personal fall arrest, exposing themselves to a 31.8-foot fall hazard.
