29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about December 6, 2012, on the 4th floor, an employee was working near an unprotected edge without the use of guardrail systems, safety nets, or personal fall arrest, exposing himself to a 40-foot fall hazard.      b. On or about December 6, 2012, on the 2nd floor, an employee was setting concrete form shoring near an unprotected edge without the use of guardrail systems, safety nets, or personal fall arrest, exposing himself to a 20-foot fall hazard.      c. On or about December 6, 2012, on the 2nd floor, 2 employees were welding steel columns in place near an unprotected edge without the use of guardrail systems, safety nets, or personal fall arrest, exposing themselves to a 20-foot fall hazard.      d. On or about December 6, 2012, on the 2nd floor, 2 employees were sweeping and chipping concrete splatter off of the deck near unprotected edges without the use of guardrail systems, safety nets, or personal fall arrest, exposing themselves to a 20-foot fall hazard.

29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a. On or about December 6, 2012, on the 2nd floor, there were several rectangle stair way holes approximately 8 by 10 feet wide, exposing employees to a 10-feet fall hazard.

On or about December 6, 2012, on the 2nd floor, near the job made ladder access there was a  rectangle hole 8 by 8 inches wide unprotected, exposing employees to tripping hazard.
