29 CFR 1910.180(h)(1)(i): No crane shall be loaded beyond the rated load, except for test purposes as provided in paragraph (e) of this section. On or about August 23, 2012, and at times prior thereto, on board the vessel USS Adventurer, employees involved in shipbreaking activities were exposed to struck-by, caught-between, and crush hazards when the crane was loaded beyond its rated load causing tracks of the Link-Belt LS-518 to rise from the ground.

29 CFR 1910.180(h)(3)(iv): Side loading of booms was not limited to freely suspended loads. Cranes were used for dragging loads sideways. On or about August 23, 2012, and at times prior thereto, on board the vessel USS Adventurer, employees involved in shipbreaking activities such as, but not limited to, cutting, torching, and securing loads were exposed to struck-by, crushed, caught-between hazards when loads were dragged across the bow of the ship.

29 CFR 1910.180(g)(1)(i): Monthly, dated and signed inspection reports on rope condition (performed by an appointed or authorized person) for truck crane(s) were not readily available showing inspection for reduction of rope diameter below nominal diameter due to loss of core support, internal or external corrosion, or wear of outside wires: On or about August 23, 2012, and at times prior thereto, during shipbreaking activity on board the vessel USS Adventurer, employees were exposed to struck-by, and crush hazards when the crane wire rope while still in service has exceeded OSHA criteria for boom hoist rope. Lack of lubrications has caused corrosion.
