The guard was located on the floor next to the machine.  b) In the Die Cast Machine Department next to DCM #24 there was a 10 foot conveyor belt that did not have guards installed over the edges of the belt.   c) In the High Procession Area on the Mazak FH 580 CNC Machine had the access door on the rear of the machine opened and the interlock had been by-passed.   d) In the High Procession Area there were 7 Jorgensen Chip Conveyor belts that were missing guard covers over the ends of the ejection area.   e) In the secondary work department on the Burr King Model 482 Three Wheel Grinder was missing a guard over the sanding belt.   f) In the High Procession Area on the #301 Mazak CNC machine Model #FH 580, interlocked barrier guards were removed.  g) In the High Procession Area on the #302 Mazak CNC machine Model #FH 580, interlocked barrier guards were removed.  h) In the High Procession Area on the #303 Mazak CNC machine Model #FH 580, interlocked barrier guards were removed.   i) In the High Procession Area on the #304 Mazak CNC machine Model #FH 580, interlocked barrier guard removed.  j)  In the High Procession Area on the #305 Mazak CNC machine Model #FH 580, interlocks had been by-passed.   k)  In the High Procession Area on the #321 Mazak CNC machine Model #FH 580, interlock barrier guards were removed.  l) In the High Procession Area on the #323 Brother CNC Machine Model # TC-324 had the interlock on the doors by-passed.
