29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous:    Near the middle interior of the house, on or about August 28, 2012, there was no grounding pin on the 3 wire extension cord plug that supplied power to a 110 VAC Black & Decker drill,.  The drill was used to ream holes in metal electrical boxes for conduit.  The drill was powered by a Coleman Powermate 6500 Watt portable generator.
