29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: The employer does not ensure all scaffold platforms are at least 18 inches wide. This violation was observed on or about August 28, 2012, when employees were exposed to a fall hazard while walking on a scaffold platform that was 10 inches wide. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds platforms and walkways are at least 18 inches wide.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: The employer does not ensure that a guardrail system is installed on all working levels of scaffolds to prevent employee injury. This violation was observed on or about August 28, 2012, when employees were exposed to a fall hazard while on a working level of a scaffold without a guardrail system in place. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds used by employees have a guardrail system installed that protects employees from falling from all open sides and ends of the platform.

This violation was observed on or about August 28, 2012, when employees were exposed to a fall hazard while using a fifteen foot tall awning beam as a working surface to install drywall without any means of fall protection. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees performing construction activities at heights greater than six feet are protected from falling.
