OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by the traveling block due to an inadequate braking system:    On about August 28, 2012, on the Franks Explorer workover rig at the Gailey #3 well site, the braking mechanism used to secure the traveling block was not inspected and maintained. As a result, the brake failed to adequately slow or stop the traveling block thereby exposing employees to a struck-by hazard.
