29 CFR 1926.404(b)(1)(ii): All 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which were used by employees did not have approved ground-fault circuit interrupters for personnel protection (NOTE: Receptacles on a two-wire, single-phase portable or vehicle-mounted generator not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.): a) Employees were using a 5000 watt Honda Model EG 5000 gas powered generator to provide electric power at the site where the outlets on the generator were not equipped with ground-fault circuit interrupters. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment used to support scaffold platforms were not specifically designed by the manufacturer for such use: a) The employer provided a JCB Loadall Model 506C rough terrain forklift to elevate a wood platform for employee use and the manufacturer did not design the equipment for such use. The platform was not approved or properly secured. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
