29 CFR 1904.40(a):     The employer did not provide an authorized government representative the records within the four business hours.      a) On or about September 10, 2012, the employer failed to provide copies of their injury and illness records for calendar year 2012 to an authorized representative.      b) On or about November 16, 2012, the employer failed to provide copies of their injury and illness records for calendar year 2012 to an authorized representative. 							    Western Roofing, Inc. was previously cited for a violation of this occupational safety 	and health standard which was contained in OSHA inspection number 313350563, citation number 2, item 1 and was affirmed as a final order on February 25, 2010, with respect to a workplace located at 3801 S. Cole, Boise, ID 83705.  								  Note:  Abatement documentation is required for this item.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     a) On August 29, 2012, an employee was exposed to fall hazards of approximately 18 feet from ground to eave and more than 30 feet from peak to ground on the rake edge while working on the roof of a two-story residence without wearing personal fall arrest equipment.							    Western Roofing, Inc. was previously cited for violations of this occupational safety and health standard which were contained in: OSHA inspection number 106543, citation number 1, item 2 and was affirmed as a final order on November 7, 2011, 	with respect to a workplace located at Lot 5, Blk 1, Cabella Creek, Meridian, ID 83680; and OSHA inspection number 518418, citation 1, item 1 and was affirmed 	as a final order on August 9, 2012, with respect to a workplace located at E. Red Rock Street & N. Weston Ave, Meridian, ID 83642.						    Note:  Abatement documentation is required for this item.
