29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about August 29, 2012, on the roof, employees personal fall arrest systems were improperly positioned. The rope grabs were positioned on the lifelines in such a manner that employees would free fall more than 6 feet (1.8 m) to the unfinished ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about August 29, 2012, employees were working from the top and/or top step of A-frame ladders. This condition exposed employees to the hazard of stumbling and falling from the ladder. Instance a: Superintendent straddled over and standing on the top rung of an A-frame ladder using pneumatic equipment to install metal roofing component. Instance b: Roofer standing and working from top rung of an A-frame ladder screwing down metal roofing component.
