(b) On or about 08/21/12 - Maintenance shop, Horizontal Band Saw, the unused portion of the blade was not guarded.     (c) On or about 08/21/12 - Mill Room, Dicer Conveyor Belt, the tail pulley was not guarded.    (d) On or about 08/21/12 - Mill Room Upstairs, Mix Compound Conveyor Belt, the tail pulley was not guarded.    (e) On or about 08/21/12 - Mill Room Upstairs, #3 Mixer, the belt and tail pulley were not guarded.

(b) On or about 08/21/12 - #1 Vinyl Extruder, the electrical panel, rated at 240 volts was not closed.    (c) On or about 08/21/12 - #7 Vinyl Extruder, the 100-Amp main electrical panel did not have a cover.    (d) On or about 08/21/12 - Roto Cure Building, the electrical panel #85 with 20 amp breakers was not closed.
