29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Pouring area, a furnace operator was exposed to molten iron while tapping the cupola and pouring iron into ladles and did not wear protective gloves, September 20, 2012 b) Pouring area, a furnace operator was exposed to molten iron while tapping the cupola and pouring iron into ladles. The operator wore pants, which had open-top pockets on the exterior, and were constructed from cloth which was not of sufficient fire rating for the operation, September 20, 2012 c) Pouring area, a furnace operator was exposed to molten iron while tapping the cupola and pouring iron into ladles. The operator did not close his fire resistant (FR) jacket while standing within close proximity to the pouring trough while moving iron was present, September 20, 2012 "ABATEMENT CERTIFICATION REQUIRED"
