29 CFR 1910.95(g)(1):     The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:      On or about October 4, 2012, at least three machine operators, two welders, and an abrasive blaster were exposed to noise above the Action Level of 85 dBA, which is a 50% dose at the 80 dBA threshold level, time weighted over the employers' work shift.  These employees were not provided audiometric testing as required by the standard.  The levels found were the following:    a) For at least one welder, the level found was 71.9% during the 458 minute sampling period.  The results include a zero increment for the 22 minutes not sampled.     b) For at least one welder, the level found was 115.2% during the 478 minute sampling period.  The results include a zero increment for the 2 minutes not sampled.     c) For at least one welder, the level found was 139.0% during the 471 minute sampling period.  The results include a zero increment for the 9 minutes not sampled.     d) For at least one welder, the level found was 175.7% during the 475 minute sampling period.  The results include a zero increment for the 5 minutes not sampled.     e) For at least one welder, the level found was 91.6% during the 296 minutes sampling period.  The results include a zero increment for the 184 minutes not sampled.      f) For at least one abrasive blaster, the level found was 237.0% during the 282 minute sampling period.  The results include a zero increment for the 198 minutes not sampled.
