29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) Facility, Hammond & Gregg Streets, Carnegie, PA:  On or about September 19, 2012, the employer did not have a written lockout/tagout program.  Employees perform maintenance on machines at the facility such as the Wellsaw band saw, Gibbons and Lewis horizontal boring mill and the Bettis walk around vertical boring mill.

29 CFR 1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:      a) Facility, Hammond & Gregg Streets, Carnegie, PA:  On or about September 19, 2012, an employee was exposed to a potential hazard while operating the pedestal grinder that had an 1/4" opening at the work rest on the right hand side that had a carbide wheel on it.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a) Facility, Hammond & Gregg Streets, Carnegie, PA:  On or about September 19, 2012, an employee was exposed to a hazard while he was operating the pedestal grinder that had a 1 1/4 inch opening between the tongue guard and the carbide abrasive wheel.

29 CFR 1910.303(b)(7)(i):  Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment        a)  Facility at Hammond & Gregg Streets, Carnegie, PA:  On or about September 19, 2012, an employee was exposed to electrical hazards due to a missing knockout to a duplex receptacle.  A pedestal grinder was plugged into this receptacle that is used by employees to sharpen tools.

29 CFR 1910.305(g)(2)(iii):  Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:      a)  Facility, Steel I Beam along the West Wall, 1st Bay, Hammond & Gregg Streets, Carnegie, PA:  On or about September 19, 2012, employees were exposed to an electrical shock hazard due to the lack of strain relief on the end of the power cord of the pedestal grinder.  The grinder is used for sharpening tools and the employees plug the grinder into the receptacle and unplug it when they are finished.
